Standard Employment Contract of the Foreign Principal/ Employer
(An agreement between the Principal and the Worker
I. Proposed Service Contract/ Recruitment Agreement
(An agreement between the Principal/Employer and Agency)
II. Copy of Valid Business License/Registration


Nos. I, II, III, & IV should be placed in company's letterhead, signed by the authorized signatory and verified by the
Philippine Embassy.

No. V, should be verified by the Philippine Embassy

Additional Requirements:
III. Authority to Withdraw the Visa and Original Visa Receipt of Payment ( for K.S.A. only)


No. VI, should be placed in company's letterhead, signed by the authorized signatory, authenticated from the
Commerce and Ministry of Foreign Affairs.
